Reflected XSS in CRM Perks Forms 1.1.0 and earlier allows remote attackers to execute scripts via crafted sample-file parameters. The publicly reachable templates/sample_file.php reads FirstName, LastName, and Company parameters and returns them without sanitization or output escaping. Exploitation requires a victim to follow a crafted link, and the injected script runs in the affected WordPress site's browser origin.
