Stored script execution via unencoded patient address

Stored cross-site scripting in Clinic's Patient Management System 1.0 allows remote authenticated users to execute script in a viewer's browser. The patients.php patient-creation path stores the address parameter and line 10 echoes it into HTML without validation or output encoding. The attack requires valid credentials to create a patient and a later patient-list page view; the script then runs in the application's browser origin.
