Duplicate transfer acceptance via unbound reference keys

Improper transfer validation in Solana Pay 0.2.0 and earlier allows remote attackers to cause one transaction to validate multiple transfers. The validateTransfer function searched the entire account-key list for each reference and used aggregate recipient balance changes without binding references to the first transfer instruction. Reachability requires an application to locate a transaction by reference and trust this result; false transfer acceptance affects integrity, with web-based point-of-sale deployments more exposed than physical devices.
