System crash via unchecked NVMe register access

Denial of service in the Linux kernel 4.19 NVMe driver allows local users to crash the system through consecutive reset ioctls. The reset path disconnects the PCIe link, then nvme_pci_reg_write32 accesses a controller register without validating that its BAR mapping remains valid. Access to an NVMe controller device file is required; rapidly issuing both reset ioctls triggers a kernel paging fault and system crash.
