Extended cluster access via unchecked token expiration

Insufficient session expiration in VMware Pinniped Supervisor 0.3.0 through 0.18.0 allows remote authenticated users to extend cluster access. The token-exchange handler validated an access token before loading its stored session, so the expiration time in that session was never checked. A legitimate user retaining an expired access token can exchange it for cluster-scoped credentials after identity-provider refresh checks would otherwise end the session.
