Unauthenticated upstream-triggered crash via gRPC health check

A denial-of-service flaw in Envoy before 1.22.1 allows remote attackers to crash the proxy via gRPC health checking. GrpcHealthCheckerImpl dereferences a null pointer after a service-discovered upstream is removed while a held host's gRPC health check fails. Exploitation requires control of both the upstream host and its service-discovery record; impact is limited to Envoy process termination.
