Authentication bypass via reusable TOTP codes

Authentication bypass in Laravel Fortify before 1.11.1 allows remote attackers to replay a valid TOTP code during its time window. TwoFactorAuthenticationProvider verifies the submitted code but does not retain its successfully used time step, so repeated submissions remain valid. Exploitation requires a target account's username and password plus a captured, still-valid TOTP code, permitting a second login as that account.
