Authenticated data access via missing endpoint ACL checks

Missing authorization in OpenEMR before 7.0.0 allows remote authenticated users to read or modify data through protected web endpoints. Numerous patient-document, billing, report, order, and practice-setting handlers execute requests without calling AclMain::aclCheckCore, while the fix inserts endpoint-specific checks. A valid account with insufficient role permissions can cross intended access boundaries to view or alter clinical, financial, and administrative records; no user interaction is required.
