Rate-limit bypass via trusted proxy header defaults

Incorrect trusted-IP-header defaults in Mattermost 6.7.0 and earlier allow remote attackers to bypass rate limits and falsify audit IPs. The ServiceSettings.TrustedProxyIPHeader default remains nonempty in affected configurations, so the server trusts request-supplied proxy headers as client addresses. No authentication is required; crafted headers can evade IP-keyed throttling or place false source addresses in audit records.
