Console account compromise via unlimited login attempts

Improper login-attempt limiting in Heroic Labs Nakama before 3.13.0 allows remote attackers to brute-force console credentials. The Console Authenticate handler checked supplied credentials without tracking failed attempts by account or client IP, so it never imposed a retry limit. The console endpoint is network reachable and requires no prior session; a guessed valid password grants the associated console account's privileges.
