Continued authenticated access via uninvalidated logout tokens

Insufficient session expiration in Heroic Labs Nakama before 3.13.0 allows remote attackers to reuse logged-out tokens for authenticated requests. The Console logout path ended the browser session without invalidating its token in the server session cache; the patch adds a logout endpoint that performs invalidation. Possession of a previously issued token is required, and its bearer retains that token's request authority until it expires.
