Guest-to-host memory access via uncleared p2m entries

Improper page-table cleanup in Xen 4.12 and later on Arm allows guest OS users to access Xen or other domains' memory. When set/way cache maintenance clears an entry's valid bit, p2m_remove_mapping, guest_physmap_remove_page, and p2m_set_entry fail to clear the still-usable mapping. The documented path requires an Arm guest to decrease its memory reservation; retained access can disclose or corrupt reassigned memory, interrupt systems, or enable privilege escalation.
