Stored JavaScript execution via unescaped rule options

Stored cross-site scripting in OpenEMR before 6.0.0.4 allows remote authenticated users to execute JavaScript through crafted rule data. The plans_config.php view concatenates rule_id and rule_title values into option-element attributes and text without context-specific escaping. A low-privilege account must create a malicious rule, and script execution occurs when another authenticated user loads the affected plan interface.
