Process crash via unchecked dyldcache mapping count

Heap-based buffer overflow in radare2 before 5.6.4 allows context-dependent attackers to crash the process with a crafted dyld cache. The dyldcache parser passes an untrusted header mapping count to va2pa even when fewer mapping entries were loaded, causing out-of-bounds heap access. Exploitation requires a user to open the malicious cache locally; the published reproducer demonstrates a crash, not code execution.
