Process crash via stale I/O bank tree pointer

Use-after-free in radare2 before 5.6.0 allows local users to crash the analysis process by opening a crafted binary file. r_io_bank_map_add_top continued red-black-tree iteration after a failed submap deletion, leaving stale node state available for dereference. Exploitation requires radare2 to process attacker-controlled input, and the publicly demonstrated consequence is process termination.
