Persistent operator access via case-sensitive de-op

Improper case handling in PMMP PocketMine-MP before 4.0.3 allows local users to retain operator permissions after de-op. Server::removeOp lowercases the supplied player name but removes only an exactly matching ops.txt key, leaving mixed-case entries intact. An existing operator entry containing uppercase letters is required, and the retained role continues to grant administrative commands until manual removal.
