Kernel panic via suspended device-mapper request queue

Race condition in the Linux kernel device-mapper request queue allows local users to trigger a kernel panic during suspend. The dm_mq_queue_rq function accepted a blk-mq request while DMF_BLOCK_IO_FOR_SUSPEND was set instead of returning BLK_STS_RESOURCE for requeueing. Reaching the observed panic requires concurrent nr_requests updates and dm-multipath suspend and resume activity; the bounded impact is a host crash.
