Service interruption via mlx5 teardown error handling

Denial of service in the mlx5 driver in Linux 5.2 through 5.15.4 and 5.16-rc1 allows local users to trigger a kernel call trace. During fast device unload, the command handler wrongly returns EIO for DESTROY_UCTX and DESTROY_UMEM after the internal-error state marks teardown as started. Reaching the fault requires local access and an mlx5 RDMA device; the documented consequence is an error trace during UMEM release and availability loss.
