Memory exhaustion via unreleased NVMe loop controllers

Memory leak in affected Linux kernel versions from 4.8 through 5.13-rc2 allows local users to exhaust memory through failed NVMe loop setup. The nvme_loop_create_ctrl() error path leaves its allocated loop controller unreleased when nvme_init_ctrl() returns an error. Reaching the leak requires local controller creation and an initialization failure; repeated attempts consume kernel memory and can interrupt service.
