Unauthenticated leader-node crash via nil envelope dereference

Denial of service in Hyperledger Fabric 1.4.0, 2.0.0, and 2.1.0 allows remote attackers to crash leader nodes via a nil payload. The orderer passes the incomplete envelope to protoutil.ChannelHeader, which accesses env.Payload without first rejecting a nil envelope. A network-reachable leader orderer that accepts the crafted message terminates with a segmentation fault, interrupting that node's service.
