Reflected XSS via unescaped request path output

Reflected XSS in Varga Zsolt Simple Popup Newsletter 1.4.7 and earlier allows remote attackers to execute scripts via a crafted URL. The simple-popup-newsletter.php file places attacker-controlled $_SERVER['PHP_SELF'] data into generated form markup without output escaping. Exploitation requires a victim to follow the crafted request, after which the script runs in that visitor's browser under the affected site's origin.
