Cluster disruption via unauthenticated Raft peer removal

Improper authentication in HashiCorp Vault Enterprise 1.6.0 and 1.6.1 allows remote attackers to remove Raft peers from DR secondaries. The Raft remove-peer update path invoked handleRaftRemovePeerUpdate without the DR operation-token verifier, so an unauthenticated request reached the peer-removal handler. Exposure requires Enterprise replication with integrated storage and a DR secondary; the final node cannot be removed, but removing other peers interrupts cluster availability.
