Stored XSS via unescaped form-name attribute

Stored XSS in CalderaWP Caldera Forms before 1.9.5 allows remote authenticated users to execute script via a crafted form name. The form builder stores the Form Name and inserts it unescaped into an HTML attribute when the form is edited, so a quote can terminate the attribute and inject markup. Administrator-level access is required even when unfiltered_html is disabled, and a subsequent visit to the form editor triggers the script in that browser.
