Stored XSS via unescaped X-Forwarded-For header

Stored XSS in Perfect Survey 1.5.2 and earlier allows remote attackers to execute script in an administrator's browser via a crafted header. The save_question_data AJAX action records the X-Forwarded-For value, and the statistics page later renders it without validation or output escaping. The survey's Anonymize IP setting must be disabled, and exploitation requires an administrator to open the affected statistics page.
