Stored script execution via unescaped form titles

Stored XSS in Form Builder | Create Responsive Contact Forms before 1.9.8.4 allows remote authenticated users to run scripts via a form title. The form-title save path stores title input without sanitization, and the administrative form view renders that value without output escaping. Administrator privileges are required to save the payload; it then runs when the stored form is viewed or edited in the dashboard, even if unfiltered_html is disabled.
