Authenticated stored XSS via unsanitized form titles

Stored XSS in Made I.T. Forms before 1.12.3 allows remote authenticated users to execute script in an administrator's browser. The Add New workflow saves the Title field without sanitizing its script markup, then renders the stored value when the plugin page is viewed. An authenticated user must create and save a form, and execution occurs when another user opens the plugin page, exposing that user's browser-session data and actions.
