Authenticated stored XSS via unsanitized form names

Stored XSS in CodePeople Calculated Fields Form 1.0.353 and earlier allows remote authenticated users to execute scripts in visitors' browsers. Users who can create or edit forms can place JavaScript in field-name or form-name inputs, which the plugin stores and renders without neutralization on administrative and public pages. Form-editing access is required, and a visitor or administrator must load an affected page for the injected script to run in that person's browser context.
