Service crash via missing Host header

Denial of service in restify-paginate 0.0.5 and earlier allows remote attackers to crash Restify web services by omitting the HTTP Host header. With hostname generation enabled by default, the middleware calls replace() on req.headers.host without checking that the header exists. Any unauthenticated request to an existing endpoint triggers the uncaught exception, terminating the Node.js process even when pagination output is unused.
