Stored script execution via unescaped form title

Stored XSS in the form-management interface of S-CMS PHP 3.0 allows remote authenticated users to execute script in an administrator's browser. The /app/form_add/ workflow stores an unescaped Title Entry value, and the recycle-bin view renders that value as active HTML and JavaScript. Administrator access is required to save the payload, and another visit to the recycle bin triggers code in that visitor's browser context.
