Guest-triggered host crash via positive error return

Denial of service in Xen through 4.13.x allows guest OS users to crash a Linux-based dom0 or backend domain via a crafted grant table. In GNTTABOP_map_grant, misplaced brackets make an error path return positive 1 instead of a negative error, which Linux grant-table code accepts as success and then uses incorrectly initialized state. Reachability requires a guest-controlled grant table, the XSA-295 fixes, and a Linux-based dom0 or driver domain; the bounded result is a crash of that backend domain.
