Backend-domain crashes via false grant-copy success

Denial of service in Xen through 4.13.x allows guest OS users to crash a backend domain through a crafted transitive grant. GNTTABOP_copy can take an incorrect continuation exit, leave per-operation status fields uninitialized, and report success without copying the grant. Exploitation requires a guest with access to transitive grants; affected callers can crash or behave incorrectly after trusting the false success result.
