Retained group privileges via skipped membership refresh

Improper privilege management in Vault and Vault Enterprise 0.9.0 through 1.3.3 allows remote authenticated users to retain revoked group access. During login or token renewal, the identity-store refresh is skipped when an authentication method returns no group aliases, leaving stale external Entity-Group mappings intact. Exploitation requires third-party authentication, group-backed policies, removal of every external group, and reauthentication; the resulting token retains the removed groups' permissions.
