Authenticated remote code execution via PHP upload

Unrestricted file upload in Fleetco Fleet Maintenance Management 1.2 and earlier allows remote authenticated users to execute server commands. The accidents_add.php upload handler accepts the value_Images_1 part as application/x-php and stores the .php payload under /files without rejecting executable content. Any authenticated account can upload a web shell and invoke it over HTTP to run commands with the web server's privileges.
